Application Checklist
  • Register for and take the High School Placement Test scheduled for Saturday, November 19, 2011.

  • Sign the Release of Records Request form and send it along with the Student Recommendation form to your child's elementary school immediately so that the school can fill out the form and submit it to CJ by December 1, 2011. Your school will send the form and school records directly to Chaminade Julienne.

  • *Complete Admissions Application form and return to Chaminade Julienne by December 1, 2011.

  • Complete PSAS form and mail it directly to PSAS in the enveleope provided with a post mark date no later than December 1, 2011. ALL parents are strongly encouraged to submit a PSAS application to be eligible for tuition assistance. Parents submitting the PSAS application by December 1, 2011 will learn how much tuition assistance they will receive upon the acceptance of their child which is decided in early January.
